Position Summary

Under the general supervision of the Director of Pharmacy Services the Clinical Pharmacist will assume the responsibility and accountability for the management of rational antibiotic therapy used or management of the critical care or high-risk patients as defined by area of responsibility. Will provide for the collaboration and consultation with other health professionals on the therapeutic use of antibiotics or other medication for patients under the scope of responsibility. Will also provide education and training for health professionals involved in this clinical forum.

Education & Experience Requirements

• Graduate from an accredited school of Pharmacy with a PharmD degree.
• Current New York State Registration.
