We are seeking a Licensed Practical Nurse (LPN) Case Manager to join our team in our Bethlehem, PA location.

The LPN Case Manager will be responsible for:

• coordinating and implementing services for consumers as identified in the service plan.
• coordinating the overall medical care for consumers, following through with Doctor's medication orders.
• provide education for better understanding of medical conditions, advocacy during medical appointments.
• providing supportive counseling, training, and case management services utilizing a flexible schedule
• on-call responsibilities.

Earn $31.84 with the selection of our Enhanced Pay option.

The ideal candidate will possess the following qualifications:

• Must be a Registered Nurse, Licensed Practical Nurse or have a bachelor's degree in nursing with a minimum of 12 credit hours in social science and a minimum of 2 years of experience in a mental health setting.
• Valid PA Driver's License
