MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury or surgery. With a comprehensive range of services supported by an integrated health system, patients can recover in the familiar surroundings of their home.
Multiple Fee for Service LPN positions within the following regions: Bronx, Manhattan, Brooklyn and Queens.
Reporting to the Coordinator of Care you will be responsible for seeing patients in their home for revisits based on a skilled need, implement the plan of care and evaluate the treatment. You will document the visit electronically and completing documentation within 24-48 hours.
Daily visit rate: $48
Weekend / Holiday visit rate: $55

- NY StateLPNLicense required
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility required.
- Must have a NY State Drivers License and a car.
- Good computer skills required.
