Employment Case Manager BHS

Assists program participants in determining their vocational/employment plans. Provides direct case management services as determined by the team. Serve as member in a collaborative, recovery-oriented, evidence based integrated employment service. Work collaboratively with clinical staff, case managers, residential team and other involved member. Carry out duties for the purpose of the development and implementation of employment strategies including job coaching within a person-centered Recovery Plan. Carry out clinical recommendations. Work in collaboration with a LMHA clinical team, track data, review progress, modify and report on individualized recovery plans as recommended by the team. Assure that project Goals are implemented for both the supported employment staff and the team on which assigned. Follow the rules and procedures of the site policies and procedures for professional services. Complete project reporting compliance with regard to data and outcomes as assigned by Director. This program is located in Stamford F.S. Debois.

ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following. Other duties may be assigned.)

  • Maintain a total caseload of 15 individuals between the ages of 18-25 requesting YAS employment services.
  • Assist program participants in determining their vocational/employment plans.
  • Conduct ongoing job development activities.
  • Maintain regular contact with area employers with the purpose of developing job leads and placements.
  • Provides on the job training (including off-site locations) to ensure that each person's individual goals on assigned caseloads are met.
  • Provide individual vocational support and updated goals of worker.
  • Assists with the coordination of daily transportation needs of participants to work by instructing on use of public transportation, tapping into other transportation resources, coordinating with other team members, etc.
  • Attends relevant team meetings, reviews and other required meetings.
  • Task analyzes skills to be taught and teach through recommended training techniques.
  • Maintains and adhere to high standards of quality control for all work produced by program participants. Maintain quality standards designated by the employer and deliver good customer service.
  • Maintains effective communication with clinicians, residential programs and other involved providers.
  • Conducts/Advocates for activities/employment that will increase economic self-sufficiency and self-image (i.e. increase workers' earnings and hours).
  • Keep workers supplied with appropriate and safe materials to ensure peak production when involved in a program setting.
Shift Details:
M/Tu/Th/F 8a-3:30p, W 8a-2:30p (w/30 min lunch)
