Part of a multidisciplinary team to deliver patient-centered nursing and rehabilitation services to patients. Responsible and accountable for planning, organizing, coordinating and evaluating the practice of MDS completion and transmission.

Responsibilities

A day in the life of a MDS Supervisor RN at Hackensack Meridian Health includes:

  • Conducts and coordinates the development and completion of the resident assessment in accordance with current rules, regulations, and guidelines.
  • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident, and family members.
  • Ensures the accuracy of diagnoses on MDS.
  • Ensures appropriate diagnoses are updated and communicated all among nursing, billing, and MDS.
  • Reviews and updates diagnoses.
  • Attends daily morning meeting.
  • Ensures MDS Department's active participation in the Utilization Review (UR) Meetings.
  • Oversees and supervises the MDS Department staff, which includes MDS Coordinators and Floating MDS a. Coordinators.
  • Provides to all professionals involved in completing the MDS (i.e. social services, dietary, nursing, rehab, activities) to assure technical competency and understanding with respect to MDS.
  • Delegates tasks and functions as necessary to the MDS Coordinators.
  • Conducts the performance evaluations of MDS personnel that are conducted at least annually.
  • Accountable for running CMS Reports, shares them and communicates findings with other team members as needed. Shares learned information to help maintain and periodically update procedures that govern the development, use and implementations of the resident assessment and care plan.
  • Reviews specific policies and procedures in the nursing manual that relate to the assessment process; and updates as necessary. Evaluate and implement recommendations from established committees as they may pertain to the assessment and/or care plan functions of the facility.
  • Is available and actively participates in all State, Federal and Joint Commission surveys. Participates in survey inspections made by authorized agencies.
  • Assists during any audit process as needed, reviews findings, documentation and makes any modification/correction of assessments if appropriate as recommended by the audit
  • Coordinates the preparation of all requests from appropriate State and/or federal regulatory agencies or agents regarding payment of services (reconsideration, denials appeals, etc.) if needed.
  • Accountable for accuracy and timely completion of Medicare certifications and re-certifications.
  • Provides written and/or oral reports of the resident assessment/care plan functions as requested or directed by committees or Director of Nursing (DON).
  • Reports all updated information to the clinical round team and DON or designated supervisor.
  • Actively participates in the weekly Utilization Review meetings if assigned Medicare residents.
  • Completes plan of corrections for any deficiencies noted on internal or external audits. Develop and participate in the planning, conducting and scheduling of timely in-service training classes that include assessment skills or techniques needed to complete the assessment functions of the facility.
  • Assesses and evaluates the outcomes of the MDS process to determine additional training, education and monitoring needs.
  • Develops and participates in the planning, conducting, and scheduling in-services, orientation, and training sections.
  • Serves as the facility resource for MDS, CMI, RUGs, and Medicare benefits and provides education to the interdisciplinary team as appropriate.
  • Maintains a current and comprehensive knowledge of MDS and Medicaid (CMI).
  • Ensures that all care plans are reviewed and updated as required by CMS regulations.
  • Monitors MDS and care plan documentation for all residents.
  • Ensures documentation is present in the medical record to support MDS coding. Participates in communicating changes of residents status to Interdisciplinary Team.
  • Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident's stay.
  • Communicates LTC Schedule with the Interdisciplinary Team.
  • Evaluates each resident's condition and pertinent medical data to determine need for unscheduled assessments.
  • Ensures that each portion of the assessment is signed and dated by the person completing that portion of the MDS.
  • Performs concurrent audits of all MDS being completed.
  • Coordinates interdisciplinary participation in completing the MDS according to OBRA and Medicare schedule.
  • Assumes responsibility for monitoring, reviewing, and transmitting resident data as part of the electronic transmission of MDS in a timely manner.
  • Reviews final validation reports and addresses any warnings, errors, or rejections as necessary.
  • Maintains all reports and transmission data in a systematic format. Demonstrates effective communication skills.
  • Communicates with DON or Supervisor if problems are not resolved in a timely basis.
  • Participates in Performance Improvement meetings. Coaches, counsels and evaluates performance of direct reports.
  • Maintains required records, reports, statistics, etc.
  • Shares information, ideas and personal skills with other team members in the department and facility. Asks if other team members need help or ask for help if you need assistance.
  • Maintains an effective, friendly working relationship with health professionals, physicians, consultants, and governmental agencies that may be involved in the resident assessment/care plan functions of the facility
  • Manages and/or assists in the recruitment, development and supervision of team members.
  • Creates and sustains an environment that fosters team member engagement. Establishes clear objectives tied to department and/or organizational strategic goals.
  • Tracks, analyzes, and reports on relevant data and metrics as needed and/or requested. Develops, recommends and/or implements actions or process improvements as appropriate.
  • Participates and represents Harborage and Nursing staff on committees, as requested by supervisors.
  • Maintains professional competence and keeps current with Nursing and Occupational Health trends through participation in continuing education, in-service programs, professional staff sessions and training of nursing personnel.
  • Fosters good relationships and communications with all Team Members, patients, family/significant others and visitors.

Qualifications

Education, Knowledge, Skills and Abilities Required:

  • Completion of an approved RN program
  • Current with all Continuing Education (CE) requirements
  • Perform clinical care professionally
  • Serve as patient advocate and role model to other Team Members
  • Delivers the highest level of service to all residents, families and visitors.
  • Deliver patient care with minimal disruptions and obstacles
  • Ability to effectively work on a multidisciplinary team

Skills and Abilities Preferred:

  • Computer skills, including Microsoft Office
  • Long-term care facility experience
  • Prior work experience with geriatric and disabled populations.

Licenses and Certifications Required:

  • NJ State Professional Registered Nurse License
