Client Summary:

Summary of Need:
Under general supervision of the Associate Director of Nursing and in accordance with Physician or Physician extender will provide education and guidance with meal planning, care coordination, nutrition evaluation and recommendation.
Job Description:
* Speaking to patients to learn about their health goals and dietary needs, preferences, and restrictions.
* Developing nutrition plans for clients.
* Compiling information and tracking client progress towards their health goals.
* Entering patient information patient database.
* Keeping informed about the latest discoveries in nutrition.
* Speaking to groups to promote healthy eating habits and proper nutrition.
Qualifications/Requirements:
Knowledge, Skills, Abilities:
Two years of experience
Competent with electronic medical charting systems -PointClick care, SigmaCare etc.;

13 weeks

- Certificate of Dietic Registration (CDR) - Required
- NY State Office of Professionals Registered License - Preferred
- Primary Source Verification - Required
- AHA BLS - Required
- Master's degree - Preferred
