Overview

Assists in the coordination of activities associated with the Asian Community Center. Functions as a liaison between external customers, patients, families and VNS Health. Builds relationships, obtains referrals and reviews patient information referred to VNS Health to determine initial appropriateness of admission. Works under general direction.


Compensation:

$20.98 - $26.23 Hourly

What You Will Do

  • Greets and introduces visitors. Provides routine information on services provided by the Center.
  • Promotes VNS Health services, including but not limited to, CHHA, Hospice, Partners in Care, and CHOICE. Follows through on various processes including completing an appropriate referral form and submitting to appropriate unit.
  • Works with physician offices or referral sources to obtain necessary information to facilitate the intake process.
  • Maintains a report that tracks various referral and admissions processed.
  • Acts as a liaison between Community Center clients and VNS Health. Facilitates communication and referrals, as needed.
  • Assists in resolving any consumers' issues; works with the appropriate departments, as needed.
  • Responsible for recruiting new members and maintaining membership database
  • Coordinates daily administrative activities of the program, such as maintenance of filing system, routine typing, photocopying, faxing, opening and routing mail, etc.
  • Develops and maintains positive working relationships with office staff. Conducts telephone outreach and surveys, as requested.
  • Maintains daily activity calendar.
  • Responsible for setting-up and breaking-down furniture from daily activities.
  • Participates in planning and coordinating of special events.
  • Monitors work assignments of volunteers.
  • Monitors, orders, and maintains an inventory of Center's supplies, literature and maintains equipment. Call for service, as necessary.
  • Participates in special projects and performs other duties as assigned.

Qualifications

Education:
High School Diploma or GED required
Bachelor's Degree preferred

Work Experience:
Minimum two years of customer service experience preferred
Experience in a healthcare organization or in social services preferred
Demonstrated knowledge of Microsoft Windows preferred

Ability to speak, read & write in Chinese required
